Immunization Updates
The Ohio Department of Health recommends a number of immunizations at various grade levels to keep all children safe from communicable diseases. Please provide the school clinic a copy of the updated immunization record from your physician when entering Kindergarten, 7th grade and 12th grade. The record can also be emailed to cworner@revereschools.org

Under some circumstances, families may receive an immunization exemption for medical or religious reasons. If this applies, please complete and submit this exemption form annually.
